SUMMARY:Webinar on Root Cause Analysis
DESCRIPTION:Description :\nRoot cause analysis (RCA) involves searchin
g backwards from an undesirable effect (or problem) to its cause(s) an
d addressing those causes. While the term \"Root Cause\" implies that
there is a single cause for a problem, more often multiple causes inte
ract and work together to trigger the problem. The RCA task is to iden
tify potential causes, determine which cause or causes are root causes
and address those root causes to ensure the effect (the problem) does
not recur. RCA is important because if a problem has occurred once, i
t most likely will occur again.\nHowever, if the root cause is found a
nd addressed, future occurrences of the same problem CAN BE PREVENTED!
RCA and corrective action is the key to preventing future problems.\n
Four RCA tools will be shared during this session, The 5 Why's, Cause
and Effect Diagramming, Contingency Diagramming and the 8D Problem Sol
ving process.\nAreas Covered in the Session :\nEstablishing an effecti
ve team environment\nClarifying what the problem is and what the probl
em is not\nSearching for the root cause by investigating the problem's
symptom(s)\nUsing data and investigative tools to identify the root c
ause(s)\nEmploying decision making to select the most appropriate solu
tion\nInitiating measurements to ensure the problem does not reoccur\n
Recognizing common barriers to RCA and how to overcome them\nWho Will
Benefit:\nQA Managers and Engineers\nManufacturing Managers and Engine
ers\nConsultants\nQuality System Auditors\nSafety and Quality Auditors
 \nProject and Program Management Teams\nOperations Management Teams\nE
