Seymens directed an initiative at Apple that developed native Macintosh clients for health IT systems. His team partnered with 120 medical schools to develop the medical apps for Macs, but they soon migrated their efforts over to Apple’s first handheld device, the ill-fated Newton, a personal digital assistant that held great promise for point of care applications...</p>
<p><br/></p>
<p>... “Of course, Apple eventually stepped back from the Newton,” Seymens told MobiHealthNews in an interview. “Honestly, it was premature at that time. What we were doing was very disruptive for physician workflow. Compare that to today: Smartphone adoption among healthcare practitioners, including Apple’s iPhone, is extremely strong.”...</p>
<p><br/></p>
<p>... "Really, this [mobile health space] hasn’t fundamentally changed all that much,” Seymens said. “Sure, the technology is better and faster. Yes, wireless is going to move it all forward, especially in the developing world because many of those markets don’t have existing infrastructure. No one is laying cable in the desert, for example.” ...</p>

<p>... Seymens is a believer in the concept of deploying mobile health solutions in developing markets and once their efficacy is proven, bringing them back to the US....</p>

<p>... Seymens left Apple in the late 1990’s to found a startup along with a few Apple Newton engineers. The startup was called JustUs Communications, which focused on creating medical apps for the Apple Newton among other devices. Again, Seymens said, JustUs was ahead of its time and it eventually ran out of gas… and its Angel funding...</p>

<p>... “Again, trying to change practice patterns for physicians is no small task,” Seymens said.</p>
<p>Soon after Seymen joined the American Hospital Association as its first Chief Information Officer and stayed on for a few years. After one too many challenges to his business acumen, Seymens said he went to Northwestern’s Kellogg School of Business to get an MBA. Seymens then joined an investment bank and headed up the firm’s healthcare technology practice and worked on capital raises, consolidations and the like...</p>

<p>... Seymens then served as president and CEO of a medical device company that retrofitted personal health devices, including pulse oximeters, weight scales, blood glucose meters and more with wireless chips. The devices could then communicate with a local hub and transmit information to secure portals where healthcare providers could analyze it and make recommendations to the patient....</p>

<p>... “I sit at that sweet spot between healthcare informatics, telecommunications and medical devices,” Seymens said. “I’m still looking very opportunistically at venture opportunities in mHealth or telehealth, but it’s a tough market for investors to get their arms around. There are not too many private equity or venture capital plays in this space. It’s very boutique-oriented with lots of one-sy, two-sy deployments. Too many proof of concept companies and no major wins yet.”...</p>

... Seymens said that mobile health won’t really take off until the telcos of the world — the AT&Ts, Verizons and Vodafones really start implementing mobile health services. Mobile operators already have the infrastructure in place, the billing in place and a captive audience, Seymens said. The mobile operators can sit back and watch the mobile health pioneers and treat them as stalking horses. My definition of ease of use: the reduction of complexity to reduce cognitive involvement ... The one vendor solution also reduces some of the problems of compatibility, e.g., apps for the iPhone are available only through the Apple iTunes website...<br/></p>

<p>... <span style="font-weight: bold;">Android</span> to the contrary has built an open source operating system to run on many phones from many manufacturers. Android is not a phone, it is an OS (Operating System). Manufacturers may license the OS for any phone that they choose. They can also change or add to the OS as needed. Android's premise is to allow developers to build apps to the specifications and needs of the end user/customer ... The developer may also choose where their apps are marketed and sold...</p>
<p>... Now for the pros and cons of both.</p>
<ul>
<li>First, both operating systems of Apple and Android are based on UNIX, an OS that was built at ATT/Bell Labs in 1969. The primary difference is the interface for both developer and user (owner of phone) ...<br/></li>
<li>The iPhone user is locked into accruing software, hardware and even music from a single vendor and store, ... A single manufacturer's hardware failure would leave an organization with few options ... With the Android solution you are not locked into a single manufacturer. At the end of 2009 there were 18 devices to choose from <a href="http://en.wikipedia.org/wiki/List_of_Android_devices">http://en.wikipedia.org/wiki/List_of_Android_devices</a>. </li>
<li>Apple's “one size fits all” solution does provide some benefits. There are not as many issues with compatibility of devices or OS, ... Android apps can have problems with apps not running on all released OS versions from different manufacturers... <br/></li>
<li>The Android user interface does take a few extra minutes to learn ... <br/></li>
<li>The only other major consideration when choosing smartphones and mobile device solutions is that you must be concerned about the carrier selection ... The iPhone is only available on AT&T (in the US) at this time. Redundancy and a single point of failure is an issue for mission critical systems. Android is available on multiple carriers such as Verizon, Sprint and T-Mobile.</li>
</ul>
<p>The iPhone and Android smartphones are basically both very good devices. The user or the institutional IT leader must examine the pros and cons for their business or personal use...</p>

<p><a href="http://www.cardionet.com/">CardioNet</a>, which has been trying to commercialize constant wireless heart monitoring for a few years now, <a href="http://www.google.com/finance?hl=en&q=BEAT&rlz=1B3GGLL_enUS369US370&um=1&ie=UTF-8&sa=N&tab=we">is at a five-year low</a>, and falling, because it can’t get insurance reimbursement for its system. <p>The latest “no,” from United Healthcare, <a href="http://english.capital.gr/news.asp?id=1006460">seems especially discouraging</a>. The insurer calls CardioNet’s solution “unproven,” preferring LOOP systems that <a href="http://allnurses.com/cardiac-nursing/can-you-explain-261633.html">are used intermittantly</a>, when a patient feels the need for monitoring. <br/></p>
<p><br/></p>
<p>It’s a distinction without a difference to the insurers, but it’s a major difference if the patient has an active lifestyle. My late journalism teacher, Richard Schwarzlose, was riding his bike when he<br/>
suffered his fatal heart attack. He needed something that could record his condition, in real time. Something he could wear comfortably...</p>
<p><br/></p>
<p>... <a href="https://www.highmarkmedicareservices.com/">Highmark CMS</a>, a northeastern payor that had been covering CardioNet, <a href="http://mobihealthnews.com/3172/cms-slashes-cardionet-reimbursement-rate/">cut its reimbursement rates</a> dramatically last year, leading to a freefall in the stock...</p>
<p><br/></p>
<p>The problem, as its then-CEO said, was that CardioNet <a href="http://mobihealthnews.com/5544/cardionet-ceo-not-able-to-sustain-operations/">could not survive</a> with the new rate, about $750 per patient.</p>
<p><br/></p>
<p>That’s where I have a problem. Technology costs continue to fall. Carrier costs can be reduced by using text messaging. To me, the CardioNet solution is overly complex, and the company is too focused on the insurance industry...</p>
<p><br/></p>
...As seen in the illustration, it’s a pretty complex deal, with three EKG leads wired to a box worn around the neck, which in turn is linked wirelessly to the phone.<br/><br/>
<p>That’s a lot to wear. It should be possible to reduce this to a patch worn close to the heart, and a smaller monitor worn on the wrist, which can carry a heart-rate monitor on it and support Zigbee to the heart and a WiFi chip connected to a smartphone app.</p>
<p><br/></p>
<p>CardioNet, in other words, is focused more on Washington than on Silicon Valley. It’s making the classic mistake of thinking more about its business model than the customer experience.</p>
<p>As a result, I think it is still missing a mass market opportunity. So is everyone else.</p>
