As reported in iHealthBeat ... Preliminary findings from the American Academy of Family Physicians' National Demonstration Project found that health IT such as electronic health records, Web portals and chronic disease registries can be implemented by a variety of practices, including small and rural offices, Healthcare IT News reports.

The two-year pilot study examined the efficacy of the patient-centered medical home model among more than 36 practices nationwide. AAFP subsidiary TransforMED conducted the project, which ended in May 2008.

In their preliminary findings, independent evaluators found that:

* 14 of 31 practices successfully implemented Web portals, including 12 small, private practices;
* 15 practices successfully established disease registries, including 10 small, private practices;
* Many small practices required assistance with EHR selection, software capabilities and other issues; and
* Some practices required assistance with implementing and integrating health IT measures.
